Enrollment Requirements

Please be sure you submit the following documents in order to complete registration:

• Completed Registration Packet

• Proof of Age and Identity for Students Proof of Age

• Immunization Records signed by a physician or an official health agency

• Proof of Residency

• Home Language Survey

• Current Court Orders with official signatures, dates and seals for Custody, Guardianship, or other Legal Documents

• Special Education Records (including IEP/Speech/Psychological Evaluations)

• Signatures on all forms that require them

* A Withdrawal Form is required if you are coming to Happy Valley School from any other school in the State of Arizona.  You will not be able to start school without this form.
