Please be sure you submit the following documents in order to complete registration:
• Completed Registration Packet
• Proof of Age and Identity for Students https://www.azleg.gov/ars/15/00828.htm
• Current Immunization Records signed by a physician or an official health agency/ Exemption form
• Proof of Residency
• Home Language Survey
• Current Court Orders with official signatures, dates and seals for Custody, Guardianship, or other Legal Documents
• Signatures on all forms that require them
* A Withdrawal Form is required if you are coming to Happy Valley School from any other school in the State of Arizona. You will not be able to start school without this form.
